Californian Doctor, Dr Gary Rheinschild faces possible disciplinary action and even a reprimand of his licence following complaints that he botched up 3 penile enlargement surgeries in the years leading to 2005.

According to the California Medical Board, the urologist, aged 75, is accused of causing gross misconduct in his treatment of these 3 men and of continuing to perform surgery despite revealing signs of cognitive impairment.
Explaining his case, Rheinschild’s lead attorney claims that these allegations against his eye sight are false, as a Judge has already proclaimed that Dr Rheinschild is not visually impaired. Continuing, Rheinschild attorney says these accusations are due to competitors looking to ruin his reputation, and nothing more.

Are their accusations real?

Penis enlargement surgeries are well known for holding an element of risk, primarily causing in penile shortening and deformity, and it could be agreed that this is what occurred here.

As Rheinschild’s attorney revealed to the press: ‘…like any cosmetic procedure, there is always somebody who doesn’t like the results. That is part of the risks and benefits that you go through.’

Penile enhancement surgery is not acknowledged as being safe or effective by the Medical Association, and unfortunately for Rheinschild the New York Times are agreeing that his penis reconstructions often ended negatively.

In the records provided by the American Urological Association, the 3 defendants announced the following.

Accusation No. 1: Client sought out Dr Rheinschild for penis enlargement surgery in 2003, and at the time expressed his concerns that heseemed a bit struggling with his memory. Described by his medical staff as being ‘eccentric’ but the best surgeon in his niche, the man underwent precedure only to later discover that his penis was deformed. Despite asking for further surgery, Rheinschild declined to help.

Client Two: 21 year old patient approaches Dr Rheinschild in early 2005. In his witness statementhe noticed that Rheinschild’s hand often quivered and his short term memory was poor. Five weeks after the $8,000 surgery, the skin flap applied to his penis burst open and his penis began to dramatically curve and shorten. After repetitively seeking out a meeting with Dr Rheinschild – who refused – the patient decided to goelsewhere to undergo cosmeticsurgery on his member.

Case Three: Following penile enlargement surgery implemented by Dr Rheinschild, patient no. 3 moaned about of pain and a large scar after skin was taken surgically removed from his lower backto create the skin flap. Accompanying this scarring the patient noticed that his skin was bruising. Despite repetitively trying to reach Dr Rheinschild continually, his messages were not returned until he threatened to take him to the Medical Board.

Will Rheinschild lose his cosmetic licence?

Since 2005, Rheinschild is noted for having performed hundreds of cosmetic surgeries without errors occurring and as a consequence, a Judge has announced against an immediate cancellation of his licence.

Further disciplinary proceedings are scheduled to occur in after Christmas.

Risk-Free Penis Enlargement

The case against Dr Rheinschild may be one of hundreds of reported instances where penile enlargement surgery has have resulted in medical complications, but THERE ARE other much safer routes for attaining your dreams of a thicker, longer penis.

Medically backed by the CE, penile enhancement gismos– such as the ones sold by SizeGenetics – are scientifically proven to permanently lengthen your penis size by 3 inches in 24 weeks.

Also constructed from medical type one materials, the SizeGenetics gadget can guaranteeyou the penile trainingyou need to treat peyronies and micropenises; correct penile curvatures by up to 70%; increase blood flow and stoppremature ejaculations.

Tamiflu as a preventive measure

Can you take Tamiflu as a preventive measure? This is a popular question that everybody is asking the world as of now. For those of the lesser informed a word about what Tamiflu is for, and what causes you to take the drug.

What Tamiflu is,where to buy tamiflu?

Tamiflu is a prescription drug for the influenza. What causes this influenza? Influenza is an infection caused by the virus. There are three types of influenza. They are the type ‘A’ influenza, the type ‘B’ influenza and the type ‘C’ influenza.

Type A is the most dangerous among these types. It affects the humans in very bad way. It has caused epidemic in various regions of the world. Some of the famous epidemics attributed to the influenza are the Spanish influenza of 1918, the Asian influenza of 1957 and the Hong Kong influenza of 1969.

Prevention methods

There are different methods to prevent the influenza. The most common among them being getting one administered the flu shot. It prevents one from getting the most common types of flu. This is an injection that has been around for decades.

Nowadays modern medicines have come into the markets which have gained popularity; the most popular among them being the Tamiflu. This is an oral medication. The medicine comes in the form of a capsule, a capsule containing seventy five milligrams of the medication. This medication is also available in the form of a suspension.

I will be discussing the dosages of the medication along with how one should be taking it for when you are taking it as a preventive measure.

How to take Tamiflu

For the medicine to be beneficial for you, you should start this medication within two days / forty eight hours of the symptoms showing up. You should take this medication twice a day; once in the morning and once in the evening. This is generally prescribed for a course of five days. Do not discontinue the course in any case. As a preventive measure you should take this medicine once a day. This can be done in any of the following ways; one tablet either in the morning or in the evening.

Medication schedule

But if you are taking this medicine in the mornings then you should continue to take this tablet only in the mornings, if in the evenings, then only in the evenings. Taking the tablet one day in the morning and the next day in the evening will reduce the effects of the tablet. Then it is not going to serve any purpose. So strictly stick to a schedule. A preferable schedule will be an evening dosage. This is recommended because there is a chance of a stomach upset on your taking this medication. So you are advised to take this medicine with food. So what better time than in the evenings! You may have to take it for a period of seven days if you have been known to be exposed to the virus or if you have worked in the virus infected area. If you are continuously working in the virus infected area then you should take it as long as you are working there. Protection of the medicine lasts as long as you are taking this drug.

There are some people out there who are having a kidney problem. For them, the prescribed medication is one tablet a day if they are known to be infected. In case they are taking it as a preventive measure then it is prescribed for them to take this medicine every alternate day.

The suspension of Tamiflu is given to children under the age of twelve years. However the recommended dosages for children under the age of twelve have not been determined in case you are giving it to him / her as a preventive measure.

Alone, provide yourself a quiet, relaxing atmosphere. Focus your attention on relaxing your body until you feel calm, centered, and comfortable. Th en imagine that you are a miniature explorer traveling around inside your body, searching for diff erent energies or feelings. Where in your body do you experience joyful feelings? In your face, eyes, mouth? In your chest or legs? Where in your body do you experience feelings of anxiety or fear? In your stomach? In your chest? In your cold hands? Where in your body do you experience anger? In your hot cheeks or ears? In your throat or neck? In your stomach? Where in your body do you experience feelings of sadness? Where in your body do you experience feelings of confusion, indecisiveness, ambivalence? Where in your body do you experience feelings of sensuality and sexual desire? Write down what you observe. Be specifi c about each feeling’s “location” in your body. What are you learning?

Are You Free to Feel?

Your feelings are valuable sources of personal information, but don’t let them run your life. It is not always a good idea to act on them—especially impulsive sexual feelings. Whether and how to act are ethical choices that you need to make. For example, a feeling of anger off ers personal information to you about your situation, usually one in which you feel frustrated, treated unfairly, misinterpreted, hurt, threatened, or blocked. Th ese feelings get your attention by agitating your body so you recognize the problem. What you do with this information is the issue. Th e guiding principle is to accept your feelings and judge your behaviors. When you make this distinction between feelings and behaviors, you are free to feel. You can feel frustration and choose not to express this feeling to your partner. Rather, you can choose a more positive course by pausing to calm your body (feelings) and then asking your partner for a few moments to cooperatively discuss the matter. You want to learn from your feelings but not let them dictate to you. You want to listen to your feelings, consider their counsel, and then decide how to respond in a constructive, eff ective fashion. Integrating your feelings and reason gives you a more complete picture of your life, relationship, and sexuality.

Return to Sexual Problems - Overview

Erectile dysfunction (ED), or impotence, is what most people think of when they hear the term “male sexual problem.” The National Institutes of Health estimates 15 million to 30 million American men do suffer from erectile dysfunction and need drugs to have sexual intercourse. However, other forms of sexual dysfunction can affect men. These include:

Hypoactive sexual desire disorder: Men with this disorder have a persistent lack of sexual desire or appetite, absence of sexual fantasies and complete lack of interest in and avoidance of sexual contact with a partner. Possible physical causes include drug side effects and hormonal deficiencies. Sometimes, boosting abnormally low testosterone levels helps. It may also be caused by or result from traumatic events in childhood or adolescence. Depression also may play a role.

Male orgasmic disorders: Also called ejaculatory disorders, these include inhibited ejaculation (orgasm does not occur) and premature ejaculation (when ejaculation occurs before, during or soon after penetration and before the man desires such as buy viagra). Inhibited orgasm is usually caused by a psychological disorder such as depression or anxiety, or use of substances like alcohol or drugs. The man’s emotional state and feelings such as guilt, boredom or resentment also may play a role. The cause of premature ejaculation is unclear but is thought to result from a combination of psychological and physical factors. Both problems are typically treated by teaching the man and his partner techniques for either producing or slowing down orgasm. In some cases, premature ejaculation can be treated with small doses of an SSRI antidepressant such as Prozac®, Paxil® or Zoloft®, taken either daily, or one to two hours before a sexual encounter.

Children, teens and adults being treated with antidepressants, particularly anyone being treated for depression, should be watched closely for worsening of depression and for increased suicidal thinking or behavior. Close watching may be especially important early in treatment, or when the dose is changed, either increased or decreased. Bring up your concerns immediately with a doctor.

Peyronie’s disease: Thought to affect about 1 percent of men usually between the ages of 40 and 60, Peyronie’s disease is characterized by the formation of a hard, fibrous layer called plaque under the skin on one side of the penis. This disorder usually starts out as an inflammation, leading to a hardened scar that causes the penis to bend sharply when erect. If hardening occurs on both sides, indentations and shortening may result. The scarring or hardening can make erections painful and intercourse difficult or impossible. The bent or misshapen appearance of the penis can lead to emotional distress, which in turn worsens any sexual difficulties. Doctors are not sure what causes Peyronie’s disease, but in many cases, the condition resolves itself. A physician will usually monitor the man closely for about a year, watching the plaque development and checking erectile function. Medications that might help to alleviate plaque buildup include topical vitamin A, collagenase ointment, B-complex vitamins or calcium channel blockers. These treatments are still unproven.

Some researchers have given vitamin E orally to men with Peyronie’s disease in small studies and have reported improvements. Yet, no controlled studies have established the effectiveness of vitamin E therapy. Similar inconclusive success has been attributed to oral application of para-aminobenzoate, a substance belonging to the family of B-complex molecules. Researchers have injected chemical agents such as verapamil, collagenase, steroids, calcium channel blockers, and interferon alpha-2b directly into the plaques. These interventions are still considered unproven because studies included small numbers of patients and lacked adequate control groups.

If treatment doesn’t work and the condition doesn’t go away on its own, surgery may be necessary. Surgeons have developed techniques for removing the plaque without affecting the proper functioning of the penis.

Dyspareunia: Men who experience dyspareunia, or pain during intercourse, usually have an underlying problem such as prostatitis (inflammation of the prostate gland) or some kind of nerve damage.

DOSAGE AND ADMINISTRATION
Adults
Levitra Tablets and Oral Suspension should be administered orally to adults as described in the Dosage Guidelines table. The determination of dosage for any particular patient must take into consideration the severity and nature of the infection, the susceptibility of the causative organism, the integrity of the patient’s host-defense mechanisms, and the status of renal function and hepatic function.

The duration of treatment depends upon the severity of infection. The usual duration is 7 to 14 days; however, for severe and complicated infections more prolonged therapy may be required.

Vardenafil should be administered at least 2 hours before or 6 hours after magnesium/aluminum antacids, or sucralfate, Videx® (didanosine) chewable/buffered tablets or pediatric powder for oral solution, other highly buffered drugs, or other products containing calcium, iron or zinc.

ADULT DOSAGE GUIDELINES
* used in conjunction with metronidazole
† Generally Vardenafil should be continued for at least 2 days after the signs and symptoms of infection havedisappeared, except for inhalational anthrax (post-exposure).
** Drug administration should begin as soon as possible after suspected or confirmed exposure. This indication is based on a surrogate endpoint, Vardenafil serum concentrations achieved in humans, reasonably likelyto predict clinical benefit.4 For a discussion of Vardenafil serum concentrations in various human populations, see INHALATIONAL ANTHRAX – ADDITIONAL INFORMATION.

Conversion of I.V. to Oral Dosing in Adults: Patients whose therapy is started with Levitra I.V. may be switched to Levitra Tablets or Oral Suspension when clinically indicated at the discretion of the physician (See CLINICAL PHARMACOLOGY and table below for the equivalent dosing regimens).

Equivalent AUC Dosing Regimens
Levitra Oral Dosage Equivalent Levitra I.V. Dosage
250 mg Tablet q 12 h 200 mg I.V. q 12 h
750 mg Tablet q 12 h 400 mg I.V. q 8 h

Adults with Impaired Renal Function: Vardenafil is eliminated primarily by renal excretion; however, the drug is also metabolized and partially cleared through the biliary system of the liver and through the intestine. These alternative pathways of drug elimination appear to compensate for the reduced renal excretion in patients with renal impairment. Nonetheless, some modification of dosage is recommended, particularly for patients with severe renal dysfunction. The following table provides dosage guidelines for use in patients with renal impairment:

RECOMMENDED STARTING AND MAINTENANCE DOSES FOR PATIENTS WITH IMPAIRED RENAL FUNCTION
Creatinine Clearance (mL/min) Dose
> 50 See Usual Dosage.
30 – 50 250 – 500 mg q 12 h
5 – 29 250 – 500 mg q 18 h
Patients on hemodialysis or Peritoneal dialysis 250 – 500 mg q 24 h (after dialysis)

When only the serum creatinine concentration is known, the following formula may be used to estimate creatinine clearance.

Men: Creatinine clearance (mL/min) = Weight (kg) x (140 - age)
72 x serum creatinine (mg/dL)

Women: 0.85 x the value calculated for men. The serum creatinine should represent a steady state of renal function.

In patients with severe infections and severe renal impairment, a unit dose of 750 mg may be administered at the intervals noted above. Patie nts should be carefully monitored.

Pediatrics
Levitra Tablets and Oral Suspension should be administered orally as described in the Dosage Guidelines table. An increased incidence of adverse events compared to controls, including events related to joints and/or surrounding tissues, has been observed. (See ADVERSE REACTIONS and Clinical Studies)

Dosing and initial route of therapy (i.e., I.V. or oral) for complicated urinary tract infection or pyelonephritis should be determined by the severity of the infection. In the clinical trial, pediatric patients with moderate to severe infection were initiated on 6 to 10 mg/kg I.V. every 8 hours and allowed to switch to oral therapy (10 to 20 mg/kg every 12 hours), at the discretion of the physician.

PEDIATRIC DOSAGE GUIDELINES
Infection Route of
Administration Dose
(mg/kg) Frequency Total
Duration
Complicated Urinary Tract or Pyelonephritis
(patients from 1 to 17 years ofage) Intravenous 6 to 10 mg/kg
(maximum 400 mg per dose; not to be exceeded even in patients weighing > 51 kg) Every 8 hours 10-21 days*
Oral 10 mg/kg to 20 mg/kg
(maximum 750 mg per dose; not to be exceededeven in patients weighing > 51 kg) Every 12 hours
Inhalational Anthrax (Post- Exposure)** Intravenous 10 mg/kg
(maximum 400 mg per dose) Every 12 hours 60 days
Oral 15 mg/kg
(maximum 500 mg per dose) Every 12 hours
* The total duration of therapy for complicated urinary tract infection and pyelonephritis in the clinical trial was determined by the physician. The mean duration of treatment was 11 days (range 10 to 21 days).
** Drug administration should begin as soon as possible after suspected or confirmed exposure to Bacillus anthracis spores. This indication is based on a surrogate endpoint, Vardenafil serum concentrations achieved in humans, reasonably likely to predict clinical benefit.4 For a discussion of Vardenafil serum concentrations in various human populations, see INHALATIONAL ANTHRAX – ADDITIONAL INFORMATION.

Pediatric patients with moderate to severe renal insufficiency were excluded from the clinical trial of complicated urinary tract infection and pyelonephritis. No information is available on dosing adjustments necessary for pediatric patients with moderate to severe renal insufficiency (i.e., creatinine clearance of < 50 mL/min/1.73m2).
